Laryngeal tumors are usually starts at the vocal cords. Vocal cords are natural barriers for the cancer spread. Unless the tumor keeps growing and spreads to the levels below and above the vocal cords it will start to spread. Vocal cord tumors can be treated endolaryngealy by LASER. The patient tends not to talk for a month for a precaution. After this period the voice box will heal and speaking can be possible. Trans-oral microscopic larynx surgery is done by Assoc. Prof. Dr. Mete İnançlı for a Glottic T2N0M0 Laryngeal Carcinoma last week. Laser gives us the advantage of less pain, less bleeding and shorter healing time.

Gırtlak reflüsü Ağızda tat bozukluğu, yutkunurken takılma hissi, geniz akıntısı, sürekli ses temizleme ihtiyacı, ses kısıklığı diş etlerinde ve dilde yanma, hassasiyet gibi birçok şikâyete neden olabilen gırtlak reflüsü günümüzde oldukça yaygın olarak görülmektedir. Pandemi döneminde vücut direncimizi yükseltmemiz için yayın organlarından yapılan gıda önerileri miktar ve çeşitlilik olarak abartılıp mide, barsak sorunları ile sıkça polikliniklere başvurmaktadırlar. Karantina tedbirlerinin akabinde artan şikâyetlerle polikliniklere başvuran hastalarda gırtlak reflüsü bulguları endoskopik görüntüleme ile tespit edilerek tanı konulmaktadır. Tanıyı tedavi ve diyet önerileri takip eder. Tedavi süreci sonunda hastaların büyük çoğunluğunda tüm semptomların ortadan kalktığı görülür. PREPARING FOR THE SURGERY
Surgery as a word can frighten you but it is good to hear that your disease has a cure by surgery. Day by day surgical interventions are transforming to more minimally invasive and comfortable. Surgery it self and post surgery recovery durations also getting shorter by technical innovations. But some of the surgical methods can remain conventional or traditional due to its effectivity and capability of supplying comforth after the surgery. Preparations for the surgery can be different for adults and for the children. That’s why in the following paragraphs they will be held in two headings.

Assoc. Prof. Dr. Mete İNANÇLI will refer you to his assigned well experienced laboratories. In every city there are laboratories that we recommend.
Preoperative Laboratory Assessments for Adults;
Complete Blood Count – Blood Glucose – ALT – AST – Urea – Creatinin – PT – aPTT – INR – Bleeding Time – Clothing Time – HIV Ab/Ag – HBsAg – Anti HCV
If our patient is over 40 year of age EKG and PA Chest X-ray is also needed preoperative.
Some other disease specific test are designed for each patient for the considered disease.
Preoperative Laboratory Assessments for Children;
Complete blood count – Bleeding time – Clothing time
We strongly try to avoid computed tomography but in some complicated infections and some serious tumoral diseases we also ask for a computed tomography. Especially for the diagnosis of sinusitis we never as for tomography! In case of a surgery decision or a complication thought we recommend first Paranasal Sinus MR Imaging.
For chronic middle ear infections if we thought that the patient might have a complication we ask for Temporal Bone Computed Tomography.
In case of a TİNNİTUS (ringing ears) we will recommend EAR MR imaging.
If the patient had a maxillofacial trauma we might ask for 3d computed tomography for to detect the form of the facial bone fractures.
For the physiologic detections of vestibular and cochlear nerve we usually recommend an audiologist to get involved in to your assessment tests for hearing and balance. We have assigned audiology laboratories for you if needed.
For detection of the adenoid vegetation in childhood X-ray is never recommended. Assoc. Prof. Dr. Mete İNANÇLI brought 2,8mm fine flexible nasopharyngolaryngoscope to North Cyprus. He is routinely applying endoscopy for his patients.
Interventions are getting more non invasive and comfortable as the technology advances.
LASER
Laser technology brings us the less pain and quicker wound healing without any scars in the mucosal levels. Ear surgery by laser is more precise for the stapedectomy and myringotomy operations. Nasal surgery by laser is much more comfortable lets us not to put any dressings in the nose after septoplasty. Inferior choncha reductions are bloodless and painless with laser. Endolaryngeal surgeries by laser are more accurate and less complicated. Endolaryngeal cordectomy and vocal cord nodule excisions can be done by laser technology. Childhood sugeries such as tonsillectomy and adenoidectomy also can be done by laser with less pain postoperatively.
Duration of the surgery by laser does not differ from the conventional methods but surgeon spends less time for the coagulation controls after the surgery.
The main advantages of laser surgery (with particular reference to the transoral laser microsurgery):
- Simplicity
- Shorter operation duration
- Minimally invasive surgery
- Lower morbidity
- Possibility of operating on elderly and /or debilitated patients and/or with pacemaker not like electro cautery.
- Shorter hospitalization or outpatient procedures
- Results of surgical oncology equal to or even better than those of traditional surgery
These laser systems are excellent surgical instruments, characterized by remarkable hemostasis ability and reduced lateral thermal damage.
Diode Laser Surgical Techniques are used in ENT, primarily in middle ear surgery, in endolaryngeal surgery, in endonasal surgery and in oral surgery.
Finally, thanks to the photomechanical action, lasers are used in ENT applications for the treatment of salivary stones (sialolithiasis).
In many cases, endoscopic laser lithotripsy is a viable minimally invasive alternative to transoral scialectomy or external sialoadenectomy procedures when too large salivary stones cannot be entirely removed from the ducts.

Nasal, Endonasal & Endoscopic Sinus Surgery: turbinate reduction (for hyperplasia of turbinates) | septoplasty | removal of nasal obstructions (e.g. stenosis), recurrent polyposis nasi & sinuses, synechiae | rhinophyma | cheloids and hypertrophic scars removals.
Tracheobronchial Tree: stenosis, nodules, polyps, tumors.
Head & Neck: resection of tumors in sub-facial and neck region.
MICRODEBRIDER (SHAVER)
Endoscopic sinus surgery is getting less invasive but some diseases like nasal polyposis advanced surgery skills are needed. Microdebrider system allows us to shave the polyps without bleeding. Surgery planning has to be done by imaging modality either MRI (preferred) or CT (not preferred due to cumulative dose for the head and neck region).
RADIOFREQUENCY
Radiofrequency is widely usable for the mucosal volumetric reductions and/or for the mucosal lesion excisions. Radiofrequency energy is not burning the mucosa or the tissues by means of this ability it allows the tissue to shrink without losing function. Scar formation is less by radiosurgery.
In North Cyprus Assoc. Prof. Dr. Mete İNANÇLI invested for all advanced surgery technology for his surgeries. In any indications for surgery listed technology can be applicable.
BALOON SINUPLASTY
Baloon sinuplasty is the most recent non invasive sinus surgery procedure all around the world. The procedure can be done outpatient. We prefer under general anesthesia but it can also be done under local anesthesia. The main principle is to widen the gates and clean inside of the paranasal sinuses. As soon as the gates stay open the chronic infection will never occur again. The effect of this procedure reaches up to five years. Patients may have redo operation after five years. Post operative recovery is shorter and there is no precautions as FESS post operative period. Less bleeding, less pain, quicker recovery, no hospitalization are the advantages of this technique. Only disadvantage of the procedure is the cost of the surgery.
